Atomic Playboy has good results with the Spam Karma plugin.

Spam Karma has been great for me so far as well. Quite honestly, it is the most complete and well rounded spam plugin out there.
Another hearty huzzah for Spam Karma. It’s managed to kill every single spam message I’ve gotten, and offers a whole host of features. It would be nice to have better blacklist management, but for a project that hasn’t been around all that long you really can’t beat it.
Every WordPress site should use Spam Karma — perhaps it could get folded into 1.4 or a later release?
